It also provides support for domestic and care activities. Il Sostegno aims to respond comprehensively and exhaustively to patients' needs, ensuring continuity of care and timely intervention in cases of urgent need.
Thanks to its collaboration with other associations, Il Sostegno strives to assist and accompany people with terminal illnesses and to support their families. ABOUT US
Our expanding team is made up of various professionals: nurses, specialists in chronic wound care, social and healthcare workers, care assistants, CRS healthcare workers, domestic workers, and more.
STAFF
Depending on the interventions required, various professionals from our Spitex service will be involved, such as nurses, geriatric assistants, social health workers, care assistants, and CRS healthcare workers. IL SOSTEGNO also collaborates with external consultants and specialized professionals such as diabetes nurses, dieticians, physiotherapists, etc.
Professional development and training
IL SOSTEGNO promotes continuous training for its staff, based on the collection and analysis of training needs.
The training aims to improve the skills of our staff in three areas: knowledge, know-how, and interpersonal skills.
Patients have the right to quality care based on UP-TO-DATE, APPROPRIATE, EFFECTIVE, AND ECONOMICAL SCIENTIFIC EVIDENCE.
Any comments regarding our work are welcome and considered a useful tool for guiding training and improving our service.
SPITEX IL SOSTEGNO was created to respond to the needs and problems of people who find themselves in situations of fragility or need and to support their families.
At Il Sostegno, the individual is at the center of our thinking and our commitment.
We believe that care must be comprehensive and meet both physical and psychological needs.
The goal is to ensure the quality of life and safety of the person and the environment in which they live, while also offering support and assistance to their family members.
We cater to users over the age of 16 with physical problems and in need of care and/or services.
The services offered can be temporary, for example following hospitalization for surgery or acute illness , or permanent for chronic, disabling illnesses or needs related to old age.
Our care focuses on maintaining and restoring the resources that enable the person to maintain or regain their desired level of independence.
PHILOSOPHY
The care philosophy applied is based on the care concepts of Virginia Henderson and Dorothea Orem, which highlight the 14 fundamental needs of human beings.
This type of nursing care involves an initial in-depth analysis to obtain a starting picture of the bio-psycho-socio-cultural characteristics and highlight the resources and problems of the individual.
A care plan is then developed in collaboration with the family and with reference to the context in which the client lives.
The specific nursing function is to assist the individual patient as a whole, in order to encourage their active participation in restoring and maintaining their autonomy and mobilizing their resources (teaching, assistance, substitution).
In agreement with the patient and their family, a care plan and objectives are defined, always safeguarding
and supporting the patient's independence and quality of life at home. Personalized care based on individual needs is guaranteed.
Services offered to users
CASE MANAGEMENT
After referral, A NURSE will visit the patient's home and carry out a thorough assessment of their needs (using an assessment tool).
Nursing and/or family assistance activities are carefully assessed and provided only with the agreement of the person themselves and their family members.
ASSESSMENT TOOL
The Service is required to define interventions based on a needs assessment tool, the RAI-HC, officially recognized by the Swiss Association of Home Care and the Conference of Health Directors. This tool supports our operators in identifying problems, finding appropriate strategies and interventions, and assigning the correct professional to each case.
BASIC CARE
Our care and assistance services are provided by staff trained and recognized by the Swiss Red Cross.
This is practical help that may be necessary in cases of physical or mental overload, illness, or injury in families, the elderly, the disabled, or people living alone.
Basic care prescribed by a doctor is covered by health insurance. The client contributes 10% of the cost.
Interventions are carried out after assessing the user's actual needs, taking into account their resources and those of their family members or caregiver.
NURSING CARE
Nursing care is provided by qualified personnel in accordance with the provisions of the LaMal Law.
This includes assistance to patients and advice to their relatives.
Care services prescribed by a doctor are covered by health insurance.
The client contributes 10% of the deductible.
Nursing services are limited to 60 hours per quarter.
In complex cases, after a thorough assessment of needs and in agreement with the doctor, the necessary documentation will be sent to the health insurance company to request additional hours.
TIME FOR TAKING ON THE CASE
Following an initial assessment, the nursing staff will determine the type of service and the time required to take charge, based on the patient's actual needs , taking into account their resources and those of their family members or caregiver.
The time established will be assessed by your health insurer, who is required to make a decision within 14 days.
Insurance coverage is provided if the insurer does not raise any objections within 14 days of receiving the notification.
Any objections will be communicated to the insured person by the insurer.
Care services are limited to 60 hours per quarter; in complex cases, after a thorough assessment of needs and in agreement with the doctor, the necessary documentation will be sent to the health insurance company to request additional hours. Any services or time not covered by compulsory basic insurance will be billed directly to the user according to the specific price list, which can also be consulted on the website.
In addition to the services covered by your compulsory basic insurance (and therefore paid for by the client), Spitex il Sostegno offers many other services, which are listed below.
If necessary, subject to agreement with the health management, we are able to provide assistance seven days a week (paid service, not covered by the LAMal).
If the customer has supplementary insurance, some services may be covered by it, subject to prior notification.
CARE PROCESS
The implementation of the care plan allows us to build an individualized framework, highlighting the client's problems and potential, limitations and resources in carrying out daily activities.
Based on the pre-established objectives, a series of specific care actions are implemented.
The care plan comprises five stages:

WOUND CARE AND PRESSURE ULCER PREVENTION
New care experiences and technologies allow us to improve the prevention and treatment of pressure ulcers, ensuring that skin lesions heal as quickly as possible according to current knowledge, minimizing scarring.
